The deadline for submission of applications (including this application, school transcripts, ACT or SAT scores, and letters of recommendation) for the majority of scholarships is May 1, 2017 for Fall Semester 2017.  The letter of recommendation form can be found here.  Click here for the full Scholarship Application PDF form.

Letters of Recommendation
  1. Submit two recommendations from school officials such as teachers or counselors who can specifically assess your academic performance. We prefer that these use the Department's recommendation form.

Additional Information
  1. Students who receive a scholarship, fellowship, or other financial aid administered by the University of Utah are required by state law (SB-81) to certify that they are eligible to receive the scholarship as a U.S. citizen or a citizen of another country who is lawfully present in the U.S. If students do not know whether they are in the U.S. lawfully, they should seek the advice of an immigration attorney before filing a scholarship/financial aid application.

    Prior to the disbursement of any scholarship/financial aid, the University of Utah is required by law to verify the recipient's citizenship status or lawful presence in the U.S. with the Department of Homeland Security (DHS). Once the University submits a student's information to DHS for verification, this federal agency may have the legal right to share the information with other government agencies.


  2. *I understand that, if I accept a scholarship, I must register for a minimum of 12 credit hours per semester, follow the outlined program (or an approved version thereof) in the University Catalog leading to a degree in Metallurgical Engineering, and maintain a 3.0 or better grade-point average each semester. Prior to registration each semester, I must meet with and get approval from the department's undergraduate advisor of my planned course schedule. I understand that failure to meet these requirements may jeopardize the scholarship.
  3. *I certify under penalty of perjury that I am either a U.S. citizen or a citizen of another country who is lawfully present within the U.S. I understand that my lawful presence within the US. will be verified through the Department of Homeland Security, prior to an award of this scholarship/ financial aid.
  4. *I certify that the information provided on this form is true to the best of my knowledge. I understand that any transcript of credits that I may provide may be reviewed by scholarship committee members.

Submit the following application materials by the deadline and make copies for your records.

  1. Unofficial copy of your high school transcript
  2. Unofficial college transcripts (if applicable)
  3. Copy of your ACT or SAT scores
  4. Two recommendation from school officials such as teachers or counselors who can specifically assess your academic performance are required. We prefer that these use the Department's recommendation form. If not, include two letters of recommendation with other submitted documents.
